Diagnosis of Constipation and Methods of Treatment

Various surveys report the incidence of constipation to range from 2 to 30%, with one large international survey pegging the number at 12% worldwide. The numbers encompass a wide range and probably reflect fundamental problems in the survey method itself. However, the bottom line is that constipation is a common condition that afflicts a substantial fraction of the population.

One of the problems associated with statistical surveying of constipation is that it’s a poorly defined condition. This translates into an impact on diagnosis and treatment. For example, one physician might believe that having a bowel movement every three days is ok, but another physician would be alarmed at the low frequency. The source of the inconsistent opinions is that bowel movement frequency is itself highly variable from person to person. This difficulty in definition is compounded by differences in patient-reported symptoms such as difficulty or pain in passing stool, or excessive time spend on the toilet.

After diagnosis, the patient and the physician must still face the task of trying to determine the fundamental cause of constipation. For some, the constellation of symptoms might suggest something such as irritable bowel syndrome (IBS). For others, incomplete control of muscles in the rectum due to trauma or injury can be a source of constipation. Yet for the largest group, there is no clear cause. This last group are classified into the “idiopathic” group.

The first step a patient diagnosed with idiopathic constipation might take is high fiber therapy, whether or not the patient’s self-reported intake of fiber seems to be sufficient. The reason for this is that high fiber therapy is accessible and gentle on the patient. There are two ways to carry out the treatment, one by increasing intake of foods high in insoluble fiber, the other by dosing with specific amounts of fiber supplements. The advantage of high fiber therapy is that the success of the therapy also confirms the diagnosis.

Beyond fiber laxatives, there are some natural supplements that are well-known: lubricant laxatives (mineral and castor oils) and herbal remedies (senna) are two examples. Remember, the safety of a natural remedy is never guaranteed. A case in point concerns aloe vera and cascara, which were used as natural constipation treatments. In 2002, the FDA banned their marketing and manufacturing exactly for safety reasons.

One other option is therapy via one of the many prescription drugs for constipation. A number of these overlap with over-the-counter laxatives. A very small handful are advanced, “targeted” drugs which affect the digestive tract in specific ways. In recent times, unique approaches such as electrode-assisted biofeedback have been studied as possible medical therapies. The field of constipation remedies seems poised for great changes in the coming decade.

Carotenoids and Their Health Benefits

There are more than 600 known carotenoids. Carotenoids are naturally occurring plant pigments that provide the orange and yellow colors we see in plants and their produce. The vibrant orange color of carrot is provided by its high carotenoid content, in fact carrots are the biggest known source of carotenoids. Of the 600 known carotenoids, about 50 of them can also be converted to vitamin A by the liver.

The most abundant carotenoid in carrots is beta-carotene. Beta-carotene is also the first carotenoid to be discovered back in 1831 by Heinrich Wilhelm Ferdinand Wackenroder, one of the most well researched. Some time later researchers found that beta-carotene was transformed into vitamin A by the body, helping to prevent the once common problem of night blindness.

So how do carrots help the eyes? Once beta-carotene is turned into vitamin A, vitamin A is then turned into another substance called rhodopsin. Rhodopsin is a purple based chemical needed by the eyes to actually see in the dark. No rhodopsin – no night vision!

Carotenoids further protect the eyes in their function as antioxidants. UV light is a source of harmful oxidants that attack healthy eye cells. Carotenoids are found in dense concentrations in the eye where they disarm these oxidants. Cataracts, glaucoma and macular degeneration are all linked to oxidant based damage.

Carotenoids are also strongly associated with cancer prevention. As antioxidants, they can protect DNA cells from oxidation damage, which is one cause of cancer. Many studies show that a high carotenoid diet can reduce the risk of getting cancer of the lungs, colon, esophagus and the prostate by up to 50%.

An abundance of carotenoids in the diet are also associated with reduced risk of developing cancers. A vast amount of research suggests that eating just one carrot a day can cut the likelihood of developing lung cancer in half. Research also provides evidence that a high consumption of carotenoids reduces the risk of developing bladder, colon, esophagus and prostate cancers by up to 50%.

Carotenoids also protect us from becoming night blind when they are transformed into vitamin A. Some of this Vitamin A in turn is converted into rhodopsin, a purple plant pigment needed in the eye to see at night. Vitamin A deficiency will inevitably lead to night blindness and eventually complete blindness if not supplied in the diet.

To increase your daily intake of carotenoids, I recommend making your own homemade carrot juice. Carotenoids, like other nutrients, always work best when taken naturally in a food source. Juicing also makes carotenoids more easily available for digestion.
